Medicare Annual Wellness Visit
A yearly check-in to stay ahead of your health
If you have Medicare, you’re eligible for a no-cost Annual Wellness Visit once every 12 months. This visit focuses on prevention and helping you stay healthy—not treating illness.
What is an Annual Wellness Visit?
Your Annual Wellness Visit is a personalized review of your health. It helps your care team understand your current health status and create or update a plan to keep you well.
What’s included:
Review of your medical and family history
List of current medications
Screening for cognitive health, depression, and fall risk
Review of vital signs (such as height, weight, blood pressure)
Preventive care recommendations and screenings
A personalized prevention plan for the next year
What’s not included:
It’s important to know this visit is not a physical exam. Medicare does not cover:
Hands-on physical exams
Diagnostic lab work or blood tests
Treatment of new or existing medical concerns
Medication adjustments for active conditions
If you have specific symptoms or health concerns, we may schedule a separate visit to address those needs.

With three medical clinics in Mariposa County, we provide a full range of family healthcare services. In addition, we host several visiting specialists on a regular basis. Fremont Family Physicians and Fremont Specialty Physicians are both located adjacent to the hospital giving our patients the benefit of immediate access to laboratory, imaging, cardiopulmonary, and emergency services should they be needed. Our third medical clinic is the Northside Clinic located in Greeley Hill.

Our provider mix includes Family Practice and Internal Medicine Physicians. Services include preventative care, vaccinations, women's health, same day appointments (call at 8am for first come, first serve basis), employment and school physicals, and all general health care needs. Counseling and psychiatry services are also available at Fremont Family Physicians.
