Location: Mariposa, California - Right on the Cusp of Yosemite National Park
We are seeking a skilled and compassionate RN Case Manager to join our team, supporting both Emergency Department and Acute Care patients. This role coordinates effective patient care across the continuum, ensures proper resource utilization, and supports discharge planning and care transitions. Located in the picturesque town of Mariposa, CA, our hospital offers a serene and fulfilling work environment, just minutes away from Yosemite National Park.

Job Summary - Under the direction of the Chief Nursing Officer:
• Assess, plan, implement, coordinate, and evaluate options
and services for ED and inpatient cases.
• Collaborate with physicians, nursing staff, social
services, and external providers to facilitate efficient
care and appropriate patient disposition.
• Lead interdisciplinary rounds and coordinate discharge
planning from day of admission.
• Evaluate patients for continued stay and medical necessity
according to CMS and payer guidelines.
• Ensure compliance with all federal, state, and payer
regulations regarding utilization management and case
documentation.
• Educate patients and families about available resources
and assist with placement, transportation, and follow-up
care as needed.
• Document all activities clearly and accurately in the
medical record and case management systems.
Minimum Qualifications:
• Current Registered Nurse (RN) license in the state of
California (required).
• Minimum 3 years of acute care nursing experience.
Emergency or Case Management experience preferred.
• Prior experience in a rural or critical access hospital
strongly preferred.
• Knowledge of CMS, Medi-Cal/Medicaid, and managed care
guidelines.
• Strong assessment, clinical judgment, and communication
skills.
• Proficiency with EMR systems and case management software.
Education:
• Registered Nurse (RN)
• Bachelor’s degree in nursing or related field
• Demonstrated advanced role in Leadership, or Clinical
Specialty.
Licensure / Certifications / Registrations:
• Current California RN license
• BLS Healthcare Provider Certification
• ACLS Certification
• PALS Certification
• TNCC
• MICN
Essential Skills:
• Familiarity with guidelines such as CMS, and CDPH, with
the ability to translate this knowledge into practice and
maintain accurate clinical documentation.
• Critical Thinking & Clinical Judgment: Ability to assess
complex patient needs and create safe, efficient care
plans.
• Communication: Excellent verbal and written communication
skills to effectively coordinate across interdisciplinary
teams and interact with patients and families.
• Organization & Time Management: Able to prioritize and
manage multiple cases in a fast-paced Acute and ED
setting.
• Problem-Solving: Creative and proactive in overcoming
discharge barriers and accessing appropriate care
resources.
• Knowledge of Regulations: Strong working knowledge of
Medicare/Medicaid rules, hospital utilization guidelines,
and discharge planning standards.
• Team Collaboration: Demonstrated ability to work
collaboratively with clinical and non-clinical teams
across departments.
• EMR & Tech Proficiency: Comfort with electronic health
records (EHR), utilization review tools, and case
management software.
