Position Summary

Under the direction of the Chief Nursing Officer (CNO), the Case Manager is responsible for coordinating patient care activities related to case management, utilization review, discharge planning, care coordination, and patient advocacy.

The Case Manager works collaboratively with patients, families, physicians, nursing staff, community agencies, and other healthcare professionals to promote quality outcomes, appropriate resource utilization, continuity of care, and safe transitions across the healthcare continuum. This position serves as a patient advocate while supporting compliance with regulatory requirements, medical necessity criteria, and organizational standards.

The Case Manager plays a vital role in helping patients navigate the healthcare system, ensuring appropriate utilization of services, facilitating discharge planning, and coordinating resources needed to support successful post-hospital outcomes.

Key Responsibilities

Case Management
Coordinate patient care from admission through discharge.

Assess patient, family, and support system needs and identify barriers to care.

Facilitate access to appropriate healthcare and community resources.

Collaborate with providers and interdisciplinary team members to support positive patient outcomes.

Monitor patient progression throughout hospitalization and assist with care planning activities.

Utilization Review
Review admissions and continued stays for medical necessity and appropriate level of care.

Support compliance with payer, regulatory, and reimbursement requirements.

Conduct concurrent reviews and assist with documentation supporting medical necessity.

Promote efficient utilization of healthcare resources while maintaining quality patient care.

Discharge Planning
Develop and coordinate individualized discharge plans.

Arrange post-acute services, referrals, equipment, and community resources as needed.

Identify and address barriers that may impact discharge readiness.

Promote safe, timely, and effective transitions of care.

Care Coordination
Facilitate communication among patients, families, physicians, nursing staff, ancillary departments, and outside agencies.

Participate in interdisciplinary rounds and care planning conferences.

Coordinate services necessary to support continuity of care.

Ensure patients and families receive education regarding treatment plans and available resources.

Patient Advocacy
Serve as an advocate for patients and families throughout the hospitalization process.

Assist patients and families in understanding healthcare options and discharge resources.

Support informed decision-making and patient-centered care.

Promote patient satisfaction and quality outcomes.

Minimum Qualifications

Education
Graduate of an accredited Registered Nursing program.

Associate Degree in Nursing required.

Bachelor of Science in Nursing (BSN) preferred.

Experience
Minimum one (1) year RN experience required.

Previous experience coordinating patient transitions of care and community resources preferred.

Licensure & Certifications
Current California Registered Nurse License required.

Current BLS Healthcare Provider certification required.

ACLS certification preferred.

Certified Case Manager (CCM) certification preferred.
