Position Summary

The Population Health Care Coordinator supports population health, quality improvement, and patient care coordination initiatives across John C. Fremont Healthcare District's Rural Health Clinic and affiliated programs.

Working under the direction of the RN, Quality & Population Health Manager, this position helps improve patient outcomes by identifying and closing care gaps, conducting patient outreach, coordinating referrals and follow-up care, supporting quality improvement initiatives, and assisting with data collection and regulatory reporting.

This role works collaboratively with providers, nursing staff, medical assistants, referral coordinators, community health workers, and other members of the healthcare team to improve preventive care compliance, support chronic disease management, and enhance overall quality performance. The Population Health Care Coordinator also serves as an important resource for patients by helping them navigate healthcare services and access available community resources.

Key Responsibilities

Identify and assist in closing preventive care and chronic disease care gaps.

Conduct patient outreach for wellness visits, screenings, immunizations, laboratory testing, and follow-up care.

Coordinate referrals, transitions of care, and follow-up appointments.

Assist patients in navigating healthcare services and connecting with community resources.

Monitor quality metrics and population health performance measures.

Support Quality Improvement (QIP) initiatives and performance improvement activities.

Collect, validate, track, and report quality and population health data.

Participate in chart reviews, audits, and regulatory reporting activities.

Document patient outreach and care coordination activities within the electronic health record (EHR).

Collaborate with providers, nursing staff, referral coordinators, community health workers, and interdisciplinary teams to support coordinated, patient-centered care.

Maintain compliance with District policies, HIPAA, CMS, and applicable state and federal regulations.

Participate in department meetings, education, and continuous improvement initiatives.

Minimum Qualifications

High school diploma or equivalent required.

Minimum two (2) years of experience in healthcare, care coordination, population health, quality improvement, referral management, patient access, community health, or a related healthcare environment required.

Three (3) or more years of related experience preferred.
Experience working with Electronic Health Record (EHR) systems required.

Associate's degree in healthcare, public health, social services, business, or a related field preferred.

Experience with patient outreach, care gap closure, quality reporting, referrals, or population health initiatives preferred.

Experience in a Rural Health Clinic (RHC), Critical Access Hospital (CAH), Federally Qualified Health Center (FQHC), Independent Practice Association (IPA), health plan, or community health setting is highly desirable.

Licensure / Certification
Community Health Worker (CHW), Medical Assistant (MA), Certified Medical Assistant (CMA), Licensed Vocational Nurse (LVN), or another healthcare-related certification preferred.

Valid California Driver's License may be required based on program needs.
