Position Summary
As a Licensed Vocational Nurse (LVN) in our Skilled Nursing Facility, you will provide direct and compassionate nursing care to residents in a long-term and post-acute care setting. Working under the supervision of a Registered Nurse and physician, you will administer treatments and medications, observe and report resident conditions, supervise Certified Nursing Assistants (CNAs), and participate in resident care planning. Your role is critical in ensuring quality care, resident safety, and dignity while maintaining compliance with professional standards and regulations

Key Responsibilities
Provide direct nursing care including treatments, procedures, and medication administration within LVN scope of practice.
Monitor residents’ health, report changes in condition, and carry out appropriate interventions as directed by the RN or physician.
Accurately document nursing care, observations, and resident responses in the electronic health record.
Assist with admissions, discharges, and transfers of residents.
Provide direction, guidance, and supervision to Certified Nursing Assistants (CNAs) to ensure quality resident care.
Collaborate with the interdisciplinary healthcare team and actively participate in resident care planning.
Support residents with activities of daily living (ADLs) while promoting independence and dignity.
Communicate with residents’ families, providing updates and support as appropriate.
Maintain compliance with infection control, safety, and facility policies at all times.

Minimum Qualifications
Current California LVN license in good standing.
Current Basic Life Support (BLS) certification.
Previous nursing experience in a long-term care or SNF setting preferred (new graduates considered)

Wage Range: Based on union placement within the listed wage range
Shift Differential: Additional compensation for night shifts
